    I am registering as a player for the Australia Baseball Tour May 2006 at a cost of $4,175 + Tax.

    I am registering as a non-player for the Australia Baseball Tour May , 2006 at a cost of $4,050 + Tax.

__I understand there is an additional charge for Australian Tax currently at $328 and there may be additional surcharges imposed by the airlines. 

__I understand payment is due in full to the travel agents at least 90 days prior to departure.  Cancellations are subject to airline and travel agent requirements.  Any funds held by Dennis McCroskey or Baseball Adventures is fully refundable at any time less and charges from the airline or travel agent.

__I understand the tour and all arrangements in Australia are being organized by various local baseball associations and the team will be managed by Dennis McCroskey of Baseball Adventures. Baseball Adventures will serve as liaison between the players and the Australian teams and associations.  All other trip arrangements in Australia are organized and run by local travel agents of Australia Travel for which Baseball Adventures will also serve as liaison.

The tour includes only the items described such as ground transportation, tours, lodging, baseball, meals and entertainment described or indicted  Any additional charges or fees, such as airport or entry fees, visas to the country are my responsibility. Airfare in the US to and from Los Angeles is not included.  Flight costs to, from and within Australia are included but there may be additional fuel or surcharges by the airlines. 

I understand it is my responsibility to have a valid passport and am a citizen in good standing. Enclosed is a deposit for $500. I understand this deposit may be  non-refundable if I cancel the trip.  Any funds held by Dennis McCroskey are refundable at any time.  Travel agents and their vendor may have limits or no refunds which will be forfeited if I cancel the trip.  Amount will be subject to their policies and requirements.  If the trip is canceled for no reason of my own, the deposit will be refunded in full at any time less any travel agent or vender fees. I understand that payment in full is due by January 15, 2006
