    I am registering as a player for the Prague-Brno-Vienna Baseball Tour July 23-July 31, 2007 at an estimated cost of $1995

    I am registering as a non-player for the Prague-Brno-Vienna Baseball Tour July 23-July 31, 2007 at an estimated cost of $1995.

    I would like a single room for myself for an additional estimated cost of $495.

I understand the tour and all arrangements in the Czech Republic are being organized by Czech Baseball Association (CBA).  The team will be managed by Dennis McCroskey of Baseball Adventures. Baseball Adventures will serve as liaison between the players and CBA  and the travel agents.  All other trip arrangements in the Republic are organized and run by Carlson Wagonlit Travel of the Czech Republic.

The tour includes only the items described such as the ground transportation, tours, lodging, baseball, meals and entertainment described or indicted.  Any additional charges or fees, such as airport or entry fees, visas to the country are my responsibility. Airfare to and from Europe is not included.

I understand it is my responsibility to have a valid passport and am a citizen in good standing. Enclosed is a deposit for $500. I understand this deposit is non-refundable if I cancel the trip within 30 days of the trip.  Cancellations prior to that date are fully refundable. If the trip is canceled for no reason of my own, the deposit will be refunded in full at any time. I understand that payment in full is due by June 1, 2007
